We selected Forth for their accredited NHS laboratories, GMC-registered doctor interpretations, and outstanding client experience. Their platform provides reliable biomarker data in a market leading dashboard, giving you a trusted clinical foundation before entering the coaching phase.

The test provides a broad assessment of your internal health, including:

It’s a comprehensive MOT for your body, offering insight into the systems that underpin daily performance.

No. This test requires a venous blood draw, which is more accurate and allows a wider range of biomarkers to be tested. Once your kit arrives, you’ll book an appointment at one of Forth’s partner phlebotomy clinics and have your sample taken by a trained professional.

Most results return within 2-3 working days after your blood draw. You’ll receive an email notification as soon as your report is ready.

Your results are interpreted by a GMC-registered doctor through Forth. They provide a clear clinical commentary visible in your Forth dashboard.
City Health Partners does not interpret, diagnose, or treat medical issues. We use your results only as prompts for reflection and goal-setting during your coaching sessions.

Forth’s clinical team follows established escalation pathways. If a result requires medical follow-up, they will flag this clearly in your report and advise you to contact your GP. Your CHP coach cannot advise on clinical matters but will help you consider how lifestyle factors relate to your goals and habits.

Once your results are marked as ready, you will receive a link to book your first coaching session.
Your coaching timeline:

● Session 2: Progress review, habit refinement
● Session 3: Consolidation, momentum, and next steps
Between sessions, you’ll have email or phone check-ins for accountability.

The coaching focuses on turning insight into action. We explore your goals, lifestyle patterns, behaviours, and performance demands, helping you develop sustainable habits that support energy, resilience, and long-term wellbeing. Coaching is not clinical advice. It’s a structured, client-led process designed to help you create meaningful, lasting change.

Yes. Your coaching conversations are confidential. Information is only shared if legally required or if there is a risk of harm.
Your health data inside Forth remains under Forth’s own privacy policy.

No. CHP does not offer clinical, diagnostic, or prescriptive advice. Any decisions about supplements or medication must be made with a qualified medical professional.

We ask for 48 hours’ notice for any rescheduled sessions.

We will review your progress, discuss next steps, and invite you to share feedback and a testimonial. Successful completion of the programme will entitle you to a FULL REFUND of the Pilot contribution.

Yes. You must be 18 or over to participate in City Health Partners programmes.
